Job Description:

Trusted Path Home Health is expanding and we are looking for a high-level Intake Coordinator Assistant to
support our Intake Department.
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Pay: $25–$35 per hour (depending on experience)
Location: In-office position

Role Purpose
This position is designed for an LVN with field experience or an experienced Home Health Intake Coordinator
who wants to move into an office-based role. You will assist the Intake Coordinator by reviewing referrals,
preparing clean charts, identifying missing or incorrect information, and helping ensure fast, compliant
admissions.
Key Responsibilities
• Review all incoming referrals for accuracy and completeness
• Identify missing documents, MD orders, and insurance information
• Assist in building clean admission charts for RN staff
• Recognize inappropriate referrals and escalate concerns
• Communicate with hospitals, SNFs, and referral sources to obtain missing documentation
• Support intake workflow efficiency and volume management
• Protect compliance and billing integrity by ensuring chart readiness

Job Requirements:

Qualifications
Option 1: Active LVN license with Home Health field experience
Option 2: Minimum 1 year experience in Home Health Intake
• Strong understanding of Medicare Home Health requirements
• Organized, fast, and detail-oriented
• Comfortable working in a high-volume environment
• Strong communication skills
