Job Description:

As our agency continues to grow, we are seeking a dedicated and experienced LVN Care Coordinator to join our home health team. The LVN Care Coordinator will work under the supervision of the Director of Nursing (DON) and will be responsible for coordinating patient care services, assisting with clinical operations, communicating with physicians and field staff, and ensuring timely and accurate documentation.

Job Requirements:

Qualifications:
• Current LVN/LPN license
• Home health experience
• Strong organizational, communication, and multitasking skills.
• Knowledge of home health regulations and documentation requirements is a plus.
• Ability to work independently and as part of a collaborative team.
